The Health District is a separate governmental entity. We are proud to be able to offer these valuable services to our residents. The mission of the Berwyn Public Health District is to provide medical and sanitation resources to meet the needs of our community.


A food permit is required for events that will have any food or beverages.
This form can be found in our Downloads section. The temporary food service application must be filled out 2 weeks before the event date. Once your permit has been processed and approved you may pick up the certificate at our office. Place the certificate in view of the public during the event.


JOB POSITIONS AVAILABLE

COVID-19 Liaison:
Duties include, but limited to:
- Participate in training /retraining by Safeguards Surveillance
- Adhere to protocols set in place for specimen collection, including but not limited to appropriate patient identification, registration, specimen collection, labeling, requisitioning, and transport of specimens to the lab by courier
- Assist in creating protocols and logistics
- Set-up and preparing the testing site
- Check-in test takers and directing folks arriving to the health department
- Track and Report key data points
- Comply with all designated safety policies and procedures in the work area including the use of all applicable Protective equipment when necessary to prevent exposure to potential infectious agents
- Provide patient education and excellent customer service related to COVID-19 and instructions for receiving results
- Assist with set up and breakdown of testing sites, traffic control, and maintaining flow for each facility and/or event
- Maintain all HIPPA standards
- Flexible and able to operate in any role assigned throughout the day including but not limited to registration, specimen collection, and traffic etc as assigned.
- Assisting with inventory counts and ordering supplies
- Assist with other Berwyn Public Health Department Initiatives
- Assist with other Berwyn Public Health Department Departments
- Other duties as needed
Qualifications:
- Have prior customer service experience. Having an assertive and customer friendly style of communicating.
- Organized and reliable
- Tech savvy (tools used: Apple and PC laptops, barcode scanners, etc.)

Berwyn Township Operations Manager
The Berwyn Township Operations Manager manages the daily operations of the Berwyn Township and supports operations of the Health District and Assessor’s Office by interacting and engaging closely with employees to identify any issues and provide valuable solutions, and by performing a wide variety of administrative tasks. The Operations Manager (OA) reports directly to Township Supervisor. The OA is expected to attend and provide reports to applicable government boards and manage the internal and external communications. The OA also oversees the implementation and management of any organization-wide initiatives and programs by working with staff to devise strategies and policies to meet the organizations objectives and goals.
To be successful in this role, one must be well-organized, have great time management skills and be able to act without guidance. One must also be able to flexible and fill in gaps in support or workforce.

Responsibilities

● Act as the point of contact among elected officials, employees, clients and other external partners such as vendors
● Manage information flow in a timely and accurate manner
● Oversee the performance of other clerical staff
● Maintain personnel files and manage applicable employee benefits
● Manage travel and conference/training arrangements for elected officials
● Attend applicable classes and keep apprised of current events
● Direct contact for external Accountant and Auditor
● Payroll, direct deposit, banking, petty cash, monthly bank reconciliations, accounts payable, expenses and other financial tasks
● Assist in formatting information for internal and external communications -- memos, emails, presentations, reports
● Ensure creation and distribution of board packets and attendance of board meetings
● Screen and direct phone calls, assist when and where is possible
● Organize and maintain the office filing system
● Manage disposal of records on annual basis
● Order all non-medical and office supplies
● Organize and coordinate staff meetings
● Maintain minutes from staff meetings
● Manage any building maintenance projects or issues as they arise
● Assist in marketing of events and manage exterior signage
● Manage log of staff duties and expectations with clear and defined roles
● Manage and update employee handbook
● Other duties as assigned

Requirements
● A bachelor’s degree from an accredited University
● Solid work experience in managerial position
● A working knowledge of government regulations
● Excellent MS Office knowledge
● Outstanding organizational and time management skills
● Excellent verbal and written communication skills
● Strong leadership in teamwork abilities
● Good interpersonal and decision-making skills

Appointments needed for all services that we provide

  • Immunizations, including flu and pneumonia vaccines
  • Low cost school and sport physicals (This is a referral program through Alivio Medical Center)
  • Lead testing
  • Tuberculin (TB) skin testing for adults and children
  • Visits to homebound for Flu/Pneumonia immunizations
  • Blood pressure checks
  • Mandated vision/hearing screenings for preschools and day care facilities
  • Free health screenings during our health fairs (This service has been suspended due to the COVID-19 outbreak)
  • Health information for various conditions/diseases
  • Rx Take Away program (drug disposal for non-controlled substances)
  • Sharps collection program (disposal of used syringes/needles)
  • ALL Kids application assistance
  • Access to Care enrollment assistance (See information below)

Access To Care Registration Site

Access to Care® is a model primary health care program serving low-income uninsured individuals in suburban Cook County, Illinois and in northwest Chicago (west of Pulaski Road AND north of North Avenue).

The Access to Care program is a unique public/private partnership making primary health care and the ancillary pharmacy, laboratory and radiology services available to those individuals caught in the gap between eligibility for public health insurance programs (All Kids, FamilyCare, Medicaid, and Medicare) and having private insurance. The program provides affordable diagnosis and treatment to individuals and families for a small co-payment per visit, procedure or prescription medication.

Eligibility screening is determined by the following criteria: family incomes below 300% of the federal poverty level, with no health insurance (or a deductible of $500 or more per person) ineligible for All Kids, FamilyCare, Medicaid, or Medicare and residence in suburban Cook County or northwest Chicago.

Residents may apply for the Access To Care Program at the Berwyn Township offices. Applications are sent to the Access to Care offices. Please call 788-6600 for an appointment or email medical@berwyntownship.org. You may also visit the Access to Care website for more information. The Berwyn Township Public Health District does not receive updates on the status of your application after it is sent the the Access to Care Offices. 


Vital Records

A driver's license or picture ID must be presented at the time of pick-up. Birth certificates are $15 for the first Certified copy and $4 for each additional Certified copy. Death certificates are $17 for the first Certified copy and $6 for each additional Certified copy. We accept Cash & Credit/Debit

Meeting Dates & Closures
