The Health District is a separate governmental entity. We are proud to be able to offer these valuable services to our residents. The mission of the Berwyn Public Health District is to provide medical and sanitation resources to meet the needs of our community.
A food permit is required for events that will have any food or beverages.
This form can be found in our Downloads section. The temporary food service application must be filled out 2 weeks before the event date. Once your permit has been processed and approved you may pick up the certificate at our office. Place the certificate in view of the public during the event.

Appointments needed for all services that we provide
- Immunizations, including flu and pneumonia vaccines
- Low cost school and sport physicals (This is a referral program through Alivio Medical Center)
- Lead testing
- Tuberculin (TB) skin testing for adults and children
- Visits to homebound for Flu/Pneumonia immunizations
- Blood pressure checks
- Mandated vision/hearing screenings for preschools and day care facilities
- Free health screenings during our health fairs (This service has been suspended due to the COVID-19 outbreak)
- Health information for various conditions/diseases
- Rx Take Away program (drug disposal for non-controlled substances)
- Sharps collection program (disposal of used syringes/needles)
- ALL Kids application assistance

Access To Care Registration Site
Access to Care® is a model primary health care program serving low-income uninsured individuals in suburban Cook County, Illinois and in northwest Chicago (west of Pulaski Road AND north of North Avenue).
The Access to Care program is a unique public/private partnership making primary health care and the ancillary pharmacy, laboratory and radiology services available to those individuals caught in the gap between eligibility for public health insurance programs (All Kids, FamilyCare, Medicaid, and Medicare) and having private insurance. The program provides affordable diagnosis and treatment to individuals and families for a small co-payment per visit, procedure or prescription medication.
Eligibility screening is determined by the following criteria: family incomes below 300% of the federal poverty level, with no health insurance (or a deductible of $500 or more per person) ineligible for All Kids, FamilyCare, Medicaid, or Medicare and residence in suburban Cook County or northwest Chicago.
Residents may apply for the Access To Care Program at the Berwyn Township offices. Applications are sent to the Access to Care offices. Please call 788-6600 for an appointment or email medical@berwyntownship.org. You may also visit the Access to Care website for more information. The Berwyn Township Public Health District does not receive updates on the status of your application after it is sent the the Access to Care Offices.
Vital Records
A driver's license or picture ID must be presented at the time of pick-up. Birth certificates are $15 for the first Certified copy and $4 for each additional Certified copy. Death certificates are $17 for the first Certified copy and $6 for each additional Certified copy. We accept Cash & Credit/Debit
